Symptoms of Distraction
1. My child does not pay attention to details or often makes carless mistakes when studying or doing other activities.
2. My child cannot concentrate on studying, work, or recreational activities.
3. My child does not seem to be listening when I am talking.
4. My child does not follow instructions and cannot complete assignments or tasks.
5. My child often has trouble getting organized for tasks or activities.
6. My child often avoids, dislikes, or rejects work that requires concentration.
7. My child often loses items necessary for tasks or activities.
8. My child often gets distracted when there are external stimulants.
9. My child often loses his daily items.
Symptoms of Hyperactivity – Impulsivity
1. My child does not pay attention to details, and often makes careless mistakes when studying, working, or during other activities.
2. My child often roams around classrooms or in situations where he/she needs to be still.
3. My child runs or climbs at inappropriate times.
4. My child cannot play quietly or enjoy leisurely activities.
5. My child is active without resting or acts as if he/she runs on a motor.
6. My child talks too often.
7. My child answers questions before the question is asked.
8. My child has trouble waiting in line.
9. My child is aggressive towards others.
